It appears the plugin may have been run with a trailing slash after the New URL but not the Old URL. Running the plugin in this way will create a number of double slashes “//” in the database.
Double slashes may be fixed by running the plugin again with a double trailing slash as the Old URL and a single trailing slash as the New URL. I have put the suggested settings in the private information for fixing the double slash.
The plugin has built in testing for settings with extra trailing slashes which may be used to prevent these types of issues in the future. More information may be found here.
Beyond the double slash, there were likely some URL missed due to the URL scheme being included in the settings. I have added suggested settings in the private information to resolve any missed URL. More information may be found here.
